ABSTRACT

ObjectiveTo assess the value of apparent diffusion coefficient (ADC) in the treatment of uterine fibroid using magnetic resonance guided focused ultrasound surgery (MRgFUS). MethodsThe MRI and clinical data of 56 patients with uterine fibroid before, at 3 and 6 months after MRgFUS treatment, at Foshan Hospital of Traditional Chinese Medicine from December 2018 to October 2022, were retrospectively analyzed. The correlation between the ADC value and lesion volume, symptoms severity score (SSS) and uterine fibroid symptoms quality of life questionnaire (UFS-QOL) were analyzed. ANOVA was used to compare the differences in related parameters before and after treatment, and Pearson’s method was performed to analyze data correlation. ResultsThere were significant differences in ADC value [(1.11±0.13), (1.84±0.09), (2.12±0.24),×10-3/(mm2/s)], lesion volume (102±35.30, 56.70±18.88, 46.93±18.99,cm3), SSS (36.73±11.74, 21.77±10.21, 17.66±9.30) and UFS-QOL score (59.05±17.48, 76.54±16.50, 82.46±12.37) between before treatment and each time point after treatment (F value was 557.837, 73.589, 53.976 and 37.606, respectively, all P<0.05). The ADC values were negatively correlated with lesion volume and SSS, and positively correlated with UFS-QOL score, with correlation coefficients of -0.586, -0.630 and 0.592, respectively (all P<0.05). ConclusionThe ADC value has clinical significance for the treatment of uterine fibroid using MRgFUS.

ABSTRACT

Objective:To systematically evaluate the efficacy and safety of filiform fire needle combined with tacrolimus ointment in the treatment of vitiligo. Methods:The RCTs of combination of filiform fire needle and tacrolimus ointment on the treatment of vitiligo were searched in the Chinese Biomedical Literature Database (CBM), China Knowledge Network Full-text Database (CNKI), Wanfang Full-text Database, VIP and PubMed databases. The studies were selected with the inclusion and exclusion criteria, then data extracted, and the quality of the included studies was evaluated according to Cochrane System Evaluation Manual. Meta-analysis was performed using RevMen 5.3 software. Results:Four studies involving 218 patients were included. After analysis, it was found that the effective rate [ OR=4.42, 95% CI (1.39-14.03), P<0.05)] and the apparent efficiency [ OR=2.89, 95% CI (1.40-5.95), P=0.004] of filiform fire needle combined with tacrolimus ointment were significantly better than that of tacrolimus ointment alone. Adverse reactions werelocal slight redness, pain and itching. None of adverse reactionswas reported. Recurrence occurred in the follow-up of were reported in two studies, which found that the recurrence rate [ OR=0.17, 95% CI (0.04-0.77), P<0.05] in the combined treatment group was significantly lower than that in the control group. Conclusions:The clinical efficacy of filiform fire needle combined with tacrolimus ointment in the treatment of vitiligo is better than that of tacrolimus ointment alone, and the recurrence rate can be reduced without any adverse reactions. However, in view of the limited number, average quality and bias of the included studies, it is suggested to carry out more multi-center, large sample and high quality randomized controlled trials for further verification.

ABSTRACT

Objective:To evaluate the efficacy and safety of Chinese medicinal formulae in the treatment of antimicrobial-resistant pneumonia. Method:Following article retrieval from eight databases and data extraction by two reviewers, the methodological quality of the included trials was assessed and the outcome indicators were subjected to Meta-analysis using RevMan 5.3. Result:A total of 24 randomized controlled trials (RCTs) were included, involving 1 818 cases. Meta-analysis showed that Chinese medicinal formulae combined with western routine intervention was superior to the western routine intervention in improving the overall response rate (ORR) [relative risk (RR)=1.27, 95% confidence interval (CI) (1.21, 1.34), <inline-formula><alternatives>P<graphic specific-use="big" xmlns:xlink="http://www.w3.org/1999/xlink" xlink:href="alternativeImage/27038DAF-2FF7-4d58-8001-0E6465A33408-M005.jpg"><?fx-imagestate width="2.28600001" height="2.62466669"?></graphic><graphic specific-use="small" xmlns:xlink="http://www.w3.org/1999/xlink" xlink:href="alternativeImage/27038DAF-2FF7-4d58-8001-0E6465A33408-M005c.jpg"><?fx-imagestate width="2.28600001" height="2.62466669"?></graphic></alternatives></inline-formula><0.000 01], the bacterial clearance rate [RR=1.49,95% CI (1.33, 1.66), <inline-formula><alternatives>P<graphic specific-use="big" xmlns:xlink="http://www.w3.org/1999/xlink" xlink:href="alternativeImage/27038DAF-2FF7-4d58-8001-0E6465A33408-M006.jpg"><?fx-imagestate width="2.28600001" height="2.62466669"?></graphic><graphic specific-use="small" xmlns:xlink="http://www.w3.org/1999/xlink" xlink:href="alternativeImage/27038DAF-2FF7-4d58-8001-0E6465A33408-M006c.jpg"><?fx-imagestate width="2.28600001" height="2.62466669"?></graphic></alternatives></inline-formula><0.000 01], and the clinical pulmonary infection score (CPIS) [mean difference (MD)=-1.64, 95% CI (-1.87, -1.41), <inline-formula><alternatives>P<graphic specific-use="big" xmlns:xlink="http://www.w3.org/1999/xlink" xlink:href="alternativeImage/27038DAF-2FF7-4d58-8001-0E6465A33408-M007.jpg"><?fx-imagestate width="2.28600001" height="2.62466669"?></graphic><graphic specific-use="small" xmlns:xlink="http://www.w3.org/1999/xlink" xlink:href="alternativeImage/27038DAF-2FF7-4d58-8001-0E6465A33408-M007c.jpg"><?fx-imagestate width="2.28600001" height="2.62466669"?></graphic></alternatives></inline-formula><0.000 01]. The comparison with the western routine intervention also revealed that Chinese medicinal formulae better improved the ORR and CPIS. Conclusion:According to the current research results, the Chinese medicinal formulae alone or combined with western routine intervention yielded more favorable clinical outcomes than western routine intervention in the treatment of antimicrobial-resistant pneumonia, without increasing the incidence of adverse events. Due to limited quality and quantity of the included RCTs, more high-quality trials are required to verify the above conclusions.

ABSTRACT

Cell apoptosis plays an important role in the development and progression of liver diseases and the maintenance of liver homeostasis. In primary biliary cholangitis, biliary epithelial cells can lead to the specific immune response of mitochondrial autoantigens. The signals such as the activation of death receptor, oxidative stress, infection, and toxin may cause selective damage of biliary epithelial cells by immune attack during cell apoptosis. Abnormal survival regulation of cells may result in a gradual loss of small bile ducts mainly in the way of cell apoptosis and may eventually lead to the development of primary biliary cholangitis.

ABSTRACT

Objective To explore the efficacy of ganoderma lucidum preparation(Ling Zhi) in treating APP/PS-1 transgenic mouse models of Alzheimer's disease(AD).Methods APP/PS-1 transgenic mice of 4 months were randomly divided into model group, ganoderma lucidum treatment groups, including high [2250 mg/(kg·d)] and middle [750 mg/(kg·d)] dose groups,i.e.LZ-H and LZ-M groups, and the positive control group(treated with donepezil hydrochloride [2 mg/(kg·d)]).In addition,C57BL/6J wild mice were selected as normal group. The animals were administered for 4 months.Histopathological examinations including hematoxylin-eosin(HE) staining, immunohistochemistry, special staining, and electron microscopy were applied, and then the pathological morphology and structures in different groups were compared. Results The senile plaques and neurofibrillar tangles in the cerebrum and cerebellum were dissolved or disappeared in LZ-H and LZ-M groups.Decrease of amyloid angiopathy was found in LZ-H and LZ-M groups. The immature neurons appeared more in hippocampus and dentate nucleus of LZ-H and LZ-M groups than those in AD model and donepezil hydrochloride groups(hippcampus:F=1.738,P=0.016;dentate nucleus:F=1.924,P=0.026),and these immature neurons differentiated to be neurons.More Purkinje cells loss occurred in AD model mice than that in LZ-H and LZ-M groups(F=9.46,P=0.007;F=9.46,P=0.010).The LZ-H and LZ-M groups had more new neuron stem cells grown up in cerebellum.Electromicroscopic examination showed the hippocampal neurons in LZ-H and LZ-M group were integrated, the nuclear membrane was intact, and the mitochondria in the cytoplasm, endoplasmic reticulum,Golgi bodies, microtubules, and synapses were also complete. The microglial cell showed no abnormality. No toxicity appeared in the pathological specimens of mice treated with ganoderma lucidum preparation. Conclusion The ganoderma lucidum preparation can dissolve and decline or dismiss the senile plaques and neurofibrillar tangles in the brain of AD mice and also reduce the amyloid angiopathy.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the clinical manifestations, diagnosis, and treatment of peripheral primitive neuroectodermal tumor (pPNET) in children and the survival of patients treated with the CCG7942/POG9354 protocol.</p><p><b>METHODS</b>A retrospective analysis was performed on the clinical data of 10 patients with pPNET admitted from October 2008 to October 2013. Of the 10 patients, 3 had metastasis, while others had no metastasis. The 7 patients without metastasis were treated with the Children's Cancer Study Group 7942 (CCG7942) protocol, and the other 3 patients with metastasis with the Pediatric Oncology Group 9354 (POG9354) protocol. The therapeutic response and chemotherapy-related toxicities were evaluated by WHO criteria and Common Terminology Criteria for Adverse Events (version 4.0).</p><p><b>RESULTS</b>In the 7 patients treated with the CCG7942 protocol, 4 achieved a complete remission (CR), 1 had stable disease, 2 developed progressive disease (PD), and 2 had recurrence. In the 3 patients treated with the POG9354 protocol, 1 achieved a CR, 2 developed PD, 2 had recurrence, and 2 died. For the 7 patients without metastasis, the survival time was 5-60 months, and the event-free survival rate was 71%. For the 3 patients with metastasis, the survival time was 13-25 months, and the event-free survival rate was 33%. All patients developed grade 4 bone marrow suppression, and other grade 1-2 toxicities, including gastrointestinal reactions, liver function impairment, and renal function impairment, were also seen.</p><p><b>CONCLUSIONS</b>CCG7942 protocol is effective and safe for children with non-metastatic pPNET. However, POG9354 protocol has unsatisfactory efficacy in children with metastatic pPNET, so further studies are needed to improve the therapy for this disease.</p>

ABSTRACT

Stellate ganglion blockade (SGB) protects patients from focal cerebral ischemic injury, and transection of the cervical sympathetic trunk (TCST) in a rat model can mimic SGB in humans. The purpose of this study was to investigate the mechanisms underlying the neuroprotective effects of TCST on neuronal damage in the hippocampus in a rat model of middle cerebral artery occlusion (MCAO) in an attempt to elucidate the neuroprotective effects of SGB. The modified method of Zea Longa was used to establish the permanent MCAO model. Male Wistar rats were randomly divided into three groups: sham-operated group, MCAO group, and TCST group. The animals in TCST group were sacrificed 48 h after TCST which was performed after the establishment of the MCAO model. Proteins were extracted from the ipsilateral hippocampus and analyzed by two-dimensional difference gel electrophoresis (2D-DIGE) and peptide mass fingerprinting (PMF). The levels of N-ethylmaleimide-sensitive factor (NSF) were measured as well. The results showed that 11 types of proteins were identified by 2D-DIGE. The expressions of eight proteins were changed both in the sham-operated and TCST groups, and the expressions of the other three proteins were changed in all three groups. Moreover, the expression of NSF was higher in the TCST group than in the MCAO group but lower in the MCAO group than in sham-operated group. The ratio of NSF expression between the MCAO group and shamoperated group was -1.37 (P<0.05), whereas that between the TCST group and MCAO group was 1.35 (P<0.05). Our results imply that TCST increases the expression of NSF in the hippocampus of adult rats with focal cerebral ischemia, which may contribute to the protection of the injured brain. Our study provides a theoretical basis for the therapeutic application of SGB to patients with permanent cerebral ischemia.

ABSTRACT

Objective To evaluate the effect of hypothermia on somatosensory evoked potentials (SSEPs). Methods Thirteen ASA Ⅱ or Ⅲ patients aged 23-51 yr weighing 45-82 kg scheduled for cardiac surgery were enrolled in this study. Bilateral median nerve SSEPs (N9, N13, N20) were recorded after induction. The MAP, peak latency and amplitude of N9, N13 and N20 were recorded when the target temperature (36, 35,34, 33 ℃ ) was reached during the cooling and rewarming periods. The neurological dysfunction was recorded after operation. Results The peak latency was prolonged and MAP was decreased with the decrease in the body temperature during the cooling period, the peak latency was shortened with the increase in the body temperature during the rewarming period ( P < 0.05), but no significant change in the amplitudes was found ( P > 0.05). The regression equation of the interaction between the peak latency and body temperature was YN9= -0.558X + 28.994(r=-0.673), YN13 =-1.121X+53.242 (r= -0.702) , YN20 = -1.458X+72.036(r= -0.702) during the cooling period (P < 0.05), and YN9 = - 0.505X + 27.313 ( r = - 0.634), YN13 = - 0.905X + 46.249(r= -0.619), YN20 = - 1.142X + 61.668 (r= -0.600) during the rewarming period (P <0.05). No neurological dysfunction was found in all the patients. Conclusion Hypothermia can prolong the peak latency of SSEP and does not alter the SSEP amplitude.

ABSTRACT

Experiment Ⅰ. Twenty-eight rats were randomly assigned into Near-Infrared Information Radiation (NIIR) group and control group. Two weeks later each rat was innoculated intraperitoneally with Salmonella typhosa H antigen (HAg) and cyclophosphamide (CY). Peripheral lymphocyte counts in the NIIR group were significantly higher than those in the control group on the fifth day after administration of CY. Experiment Ⅱ, Fifty-four rats were randomly divided into NIIR group given CY and H Ag intraperitoneally, CY and H Ag group and H Ag group for treatment. By the end of the fourth week, the survival rate and serum IgG level in the NIIR group were significantly higher than those in the CY and HAg group. By the end of 2nd week, the titer of the anti-H antibody of the HAg group and NIIR group was significantly higher than that of the CY and HAg group. Experiment Ⅲ. Thirty rats were randomly allocated to NIIR group and control group. Spleen cells were taken and cultured with Con A for 24h to induce IL-2 and the activity of IL-2 in the NIIR group was significantly higher than that in the control group. The NK activity in NIIR group was higher but not significant and ADCC in the NIIR group was significantly higher than that in the control group. The results suggest that NIIR is capable of enhancing immunoreaction in immunosuppressive bodies by promoting the function recovery of T helper cells, therefore NIIR is effective to regulate the immunological function on chronic active hepatitis.
